Clinical Lead Occupational Therapist

Leading therapy assessments and rehabilitation of patients admitted into the service primarily from the hospitals
· Access visits to the properties to determine provision for minor adaptations and equipment
· Ability to a manage a case load and delegate tasks to therapy assistants
· May need to take on referrals on the wider discharge to assess pathway
· Liaising with MDT professionals to ensure streamlined discharges
· Driving license and access to a vehicle with appropriate business insurance is essential
