Social Worker – Adult Hospital discharge team- Barnet – 32ph

Pertemps are looking for social workers to join them for an ongoing You will be working to effectively discharge adults from hospitals to home care. You will be working in the Multi disciplinary team. You must have experience of the community care act and the mental Health Act.
You will need to be registered with Social Work England
